The rental confirmation and actual contract will be mailed to you according to the information provided on this form. The rental date and time cannot be entered into the reservation book until this Rental Intent Form is returned. Dates and times are taken on a first come - first served basis. Please call ahead prior to completing this form to inquire on availability.
